AI medical scribe for doctors in India
Docna is an AI medical scribe for Indian doctors. How it works
How the AI medical scribe works
Five steps from the first word of the consult to a note in the patient's record. Only the first and the last one need you.
- 01
Choose the patient and confirm consent
Open the scribe, pick the patient or create a new one, and tap record. A consent screen asks you to confirm the patient has agreed, and gives you one line to read aloud, so the consent is part of the recording itself.
- 02
Consult the way you always do
There is no dictation voice and no script to follow. The scribe listens to the conversation between you and the patient. If you open another page mid-consult, the recording keeps running and a small pill shows it is still on. One recording can run for up to 20 minutes.
- 03
Transcription that knows Indian medicine
When you stop, the audio goes through a chain of speech models, and if one is unavailable the next takes over. The first pass is primed with Indian brands, dosing shorthand and lab names. Medicine names are then matched against more than 26,000 Indian brand and generic names, so a misheard krocin comes back as Crocin.
- 04
A SOAP note, written and then checked
The note is written only from what is in the transcript, and a section where nothing was said stays empty instead of being filled with something plausible. A separate check then reads the note against the transcript, looking for anything unsupported and anything important that was left out. A serious finding sends the note through a correction pass, and a corrected note is checked again.
- 05
You review, edit and save
The note opens section by section. Correct the chief complaint, history, examination, diagnosis and follow-up, fix a medicine's dose, frequency or duration, add or remove a medicine, then save it to the patient's consultation history, where it can be printed whenever you need a paper copy.
Capabilities
What the AI scribe writes for you
Each note follows the SOAP structure doctors already use, with the detail an Indian outpatient consult actually contains.
Subjective, objective, assessment and plan
Chief complaint, history of present illness, review of systems, and past, social and family history. Vitals and examination findings. A primary diagnosis with secondary and differential diagnoses. A plan with medicines, investigations, procedures, follow-up, patient education and referrals.
Medicines the way you prescribe them
Each medicine carries its dose, frequency and duration, under the brand you actually said. Indian shorthand such as OD, BD and TDS is understood, and so are phrases like din mein do baar or khana khane ke baad. Spellings follow Indian usage: paracetamol, not acetaminophen.
Current medicines kept apart from today's prescription
What the patient says they already take goes under current medications. Only what you start, change or explicitly continue today goes into the plan, so a tablet the patient has taken for years is listed as a current medicine, not as a fresh prescription.
The whole conversation, not only the diagnosis
Alongside the SOAP note, the scribe lists every topic you discussed, the concerns the patient raised, the counselling and reassurance you gave, why each medicine was chosen, and the warning signs you told the patient to watch for.
A check for the mistakes that matter
The verification pass looks for the errors that hurt patients: a dose with an extra zero, such as 650 heard as 6500, a dose that looks like an adult dose for a child, and any medicine in the plan that was never said in the consult.
Context from the patient's chart
Allergies, known conditions and current medicines from the patient's record go into the note-writing step, so the note is written in context. The patient's name, as written in their record, is replaced with a placeholder first, and their age is reduced to an age band.
Vitals that reach the timeline
Blood pressure, pulse, temperature, oxygen saturation and weight mentioned in the consult are captured in the note. When you save it, they are added to the patient's vitals timeline, dated to the consultation rather than to the day you saved.
The original recording, kept
The consultation audio is fingerprinted with SHA-256 in your browser, checked again on the server, and stored privately for 10 years with the saved note. You can add a trusted timestamp to a stored recording for 4 credits.
Languages
Consultations in English, Hindi and Tamil
Most OPD consults in India change language mid-sentence. The scribe detects the main language of each recording, and when it is not English it transcribes the audio again in that language, then still hands you the note in English.
English
Indian English, with Indian brand names, lab panels such as CBC, LFT, KFT and HbA1c, and dosing shorthand built into what the transcription listens for.
Hindi and Hinglish
When a consult is detected as Hindi, it is transcribed again without the English vocabulary hints, so Hindi speech comes through as Hindi instead of being forced into English. Everyday phrases are understood: bukhar is fever, din mein do baar is twice a day.
Tamil
Tamil consults are detected and transcribed in Tamil script, then written up as an English note. Before a Tamil transcript is used, Docna checks that it contains Tamil script, and when one comes back romanised, the audio goes to the next speech model instead of becoming a note.
In practice
From consult to saved note, in a real OPD
In a busy outpatient session the scribe is built to stay out of the way. Start the recording as the patient sits down and stop it when they leave. While the note is being written you can open the next patient's chart or any other page in Docna, and a notification tells you when the note is ready, anywhere in Docna.
A dropped connection does not cost you the note. If the browser loses its connection after the note was written but before you saved it, the unsaved note waits on the scribe page with the patient's name and the time, ready for you to review and save or discard. Recording works in the browser on a laptop, a tablet or a phone, including iPhone and iPad.
For video consultations there is Nova, Docna's meeting scribe. Paste a Google Meet, Zoom, Microsoft Teams or Webex link and Nova joins the call as a participant. Choose English with medical vocabulary, or Hindi and Indian languages for consults that mix Hindi and English. During the call the live view fills with symptoms, vitals, medicines and orders as they are spoken while the SOAP note drafts in the background, and you can ask Nova a question aloud by name.
After the call, ask Nova about the consultation, and its answers cite the moments in the transcript they draw on. When you are happy with the summary, file it into your consultation records in one step.

FAQ
AI medical scribe: questions doctors ask
What is an AI medical scribe?
An AI medical scribe listens to a consultation and writes the clinical note for you. Docna's scribe produces a structured SOAP note: subjective, objective, assessment and plan, with the medicines, investigations and follow-up you discussed. You review it, correct the key fields and medicines, and save it to the patient's record.
Does the AI scribe work for consultations in Hindi or Tamil?
Yes. When the scribe detects that a consultation is not in English, it transcribes the audio again without its English vocabulary hints, which keeps Hindi and Tamil speech intact. It also checks that a Hindi or Tamil transcript really contains that language's script before using it. The SOAP note itself is written in English.
Will it get Indian medicine names right?
The transcription is primed with common Indian brands and dosing shorthand such as Dolo 650, Pan D, OD, BD and TDS, and every transcript is matched against more than 26,000 Indian brand and generic names before the note is written. Each medicine is listed with dose, frequency and duration, and you can correct any of them before saving.
Can the scribe add things that were never said?
The note is written only from the transcript, and a section where nothing was said stays empty. A separate check compares the finished note with the transcript and flags anything unsupported or missing, and a serious finding triggers a correction pass and a second check. You still review every note, and can correct it, before you save it.
What happens to the patient's name and details?
Before the transcript reaches the models that write and check the note, Docna replaces the patient's name, as written in their record, with a placeholder and turns their age into an age band. Allergies, conditions and current medicines from the patient's record are included, so the note is written in context.
Can it take notes during a video consultation?
Yes. Paste a Google Meet, Zoom, Microsoft Teams or Webex link and Nova, Docna's meeting scribe, joins the call. It lists symptoms, vitals, medicines and orders live as they are spoken, answers questions you ask it by name, and after the call files its summary into your consultation records when you choose to.
How long can a recording be, and what happens to the audio?
A single recording runs for up to 20 minutes, which covers a typical outpatient consultation. The audio is fingerprinted with SHA-256 in your browser, checked again on the server and kept privately for 10 years with the saved note, and you can add a trusted timestamp to it for 4 credits.
How much does the AI scribe cost?
The scribe is included in every Docna plan. Pro is ₹4,000 a month with 30 scribed consultations a day, Enterprise is ₹8,999 a month with 100, and every new account starts with a 1-month free trial on the Pro allowance.
Finish the consult with the note already written
Start a 1-month free trial, record your next consultation in English, Hindi or Tamil, and review the SOAP note instead of typing it.
